Volunteer Background Check Forms

Citizen Schools requires background checks on all individuals who volunteer more than once in a given program season.  For returning volunteers, new background checks must be run every two calendar years.  Citizen Schools recognizes its right to terminate or discontinue the volunteer work of any individual, for any reason, at any time, with or without notice.

Volunteers have the right to dispute the accuracy of any findings revealed by the background check and to submit additional information relevant to the review of their records.  To learn more about this process, please contact Matthew Breman, National Director of Civic Engagement, at 617-695-2300 x124 or matthewbreman@citizenschools.org.

Directions:  Please print out and complete the forms for the state in which you will be volunteering.  Please return the forms to the Department of Civic Engagement.
